Consent Terms Chase
Credit Card Statement Electronic Disclosure and Consent Agreement You now have the opportunity to electronically receive Chase
OnlineSM Credit Card Statements and accompanying legal and regulatory notices ("Important Notices") which may have been included with your paper Chase Credit Card
statements. If you consent, you will stop receiving your Chase Credit Card statements and Important Notices in paper form. Important Notices may include notices regarding our privacy policies, changes in the terms of your Cardmember Agreement, reward program terms, card association benefits changes and any court notices. Certain legal and regulatory notices, such as correction notices, may be included directly on your Chase Online Credit Card Statement. At our option, we will send you Chase Online Credit Card Statements and Important Notices by any lawfully permitted electronic manner such as by posting them on the Chase Bill Management Center website, through links provided on the Chase Online Credit Card Statements or elsewhere, or by e-mail at the e-mail address you provide in the Chase Bill Management Center website. If, for whatever reason, we cannot provide you any such information in an electronic manner, at our option, we may provide any or all of such information to you by postal mail or other lawfully permitted manner. Important Notices (other than Chase Credit Card statements) are not otherwise available from us in paper or in a non-electronic form.
Upon request, you may still
obtain a paper copy of your Chase Credit Card Statement(s) by calling 1-800-323-6308;
however the Copy Fee disclosed in your Cardmember Agreement
will apply for any statements over 12 months old. In
order to receive and retain Chase Online Credit Card Statements and Important Notices, you must
have the following hardware and software equipment: Minimum

featured words and symbols used to identify the source of goods
and services are the trademarks of their respective owners. If
you have a printer, you may also print a copy of your
Chase Online Credit Card Statement(s), and those Important Notices that accompany your online statement. You can do this at no additional cost by simply clicking on the "Print"
button in your browser or, for Chase Online Credit Card Statement(s), clicking on the "Print/Save as PDF" link. You
may withdraw your consent to the electronic receipt of your
Chase Online Credit Card Statement(s) and accompanying Important Notices without terminating your Chase Bill
Management Center Service. You may do so at any time by changing
your preference via the Chase Bill Management Center Payee Info
Page or by contacting us at 1-800-201-7180. If your e-mail address
changes, you will need to promptly update it 1) by visiting the "Member Info" page within the Chase Bill Management Center website and 2) by visiting the "My Accounts" page within Chase Online and clicking on "Edit E-mail Address". You will need
to update your e-mail address at both locations to ensure the
effectiveness of the update for both services. By
clicking on the Agree button below, I confirm that I have computer
hardware and software that meets the requirements above, I have successfully accessed the sample PDF document from the link provided above, and I consent to receiving my Chase Online Credit Card Statements and accompanying Important Notices for my selected account(s) electronically as described above. I understand that by doing so, I will no longer receive
